guest list,

residence address,

contact number,

term and purpose of stay,

There should be information about whether there is a kinship relationship with the applicant.

If the inviting party is an institution; tax plate is requested.

Moreover, the inviter is required to declare in the said invitation letter that he/she will cover the subsistence and lodging of the guest under his/her own responsibility for subsistence and lodging. The applicant is required to have sufficient or regular means.
